Smoking Cessation Patient Navigator

In collaboration with patients, families and staff across all disciplines and departments, the Smoking Cessation Patient Navigator will work with the lung cancer screening program to increase access to smoking cessation services for former and current smokers. This person is responsible for supporting the goals of the program(s) and applying empowerment-based prevention and intervention strategies to improve community health specifically related to lung cancer screening, smoking cessation services, making referrals to community-based resources, addressing barriers to improving well-being, offering expertise regarding psycho-social issues affecting patient care, and educating patients regarding illness and treatment options.


  • Apply empowerment-based prevention and intervention strategies to improve community health and facilitate smoking cessation education, low dose CT (LDCT) screening, follow-up and treatment.
  • Collaborate on training for providers and staff on lung cancer screening criteria and techniques for increasing referrals to intensive smoking cessation services.
  • Organize program mailings to high-risk patient populations
  • Provide access to intensive smoking cessation services and/or lung cancer screening
  • Collaborate to ensure the timely scheduling of screening, return of results and follow up as recommended
  • Help patients requiring additional testing or treatment through patient navigation services
  • Participate in all team meetings and contribute to presentations and publications
  • Be willing to work a flexible schedule as required to meet the needs of the patient
  • Assist with data management related to lung cancer screening and smoking cessation services, including tracking of completed tests and management of testing databases.
  • Provide accurate and complete documentation of all patient contacts and patient progress towards LDCT/smoking cessation class completion.
  • Address personal and environmental issues with the patient in order to maximize emotional, social, and physical wellbeing and the appropriate use of healthcare resources.
  • Communicate and collaborate with clinic providers and support staff to assess patient’s needs and provide comprehensive healthcare.
  • Complete all direct and indirect care documentation in timely manner and ensure patient records are current and complete.
  • Attend seminars and maintain all licensure and/or certification requirements for continuing education and best practices.
  • Develop and maintain favorable internal relationships, partnerships with co-workers, including clinical managers, clinical support staff, providers and business office staff.
  • Interact respectfully and collaboratively with patients and their families, striving to developing supportive relationships

  • Medical navigation/case management 1 year preferred


  • RN or LMSW (Required)
  • NCTTS preferred or within 12 months of hire
